With more than a decade of progressive leadership at SMP, Witney Rooney has been a driving force behind the organization’s growth, integration, and success across the post-acute continuum. Over the past 10+ years, she has played a pivotal role in acquisitive expansion, program development, and the advancement of successful value-based care initiatives that strengthen patient outcomes and organizational performance.
In her role as Vice President of Post-Acute Services, Witney oversees SMP’s post-acute strategy and operations, ensuring strong connections between the acute setting, community-based partners, value-based networks, and home health agencies. She led the development of SMP’s Transitional Care Program and continues to guide key initiatives that enhance care quality, improve patient navigation, and support seamless transitions across all levels of care.
Known for her innovative approach and commitment to teamwork, Witney focuses on building collaborative models that elevate clinical performance, operational efficiency, and provider engagement. Her strategic leadership emphasizes high-quality, patient-centered care and the continuous advancement of post-acute services to meet the evolving needs of the communities SMP serves.
